Information clauses on the processing of personal data at NL Clinic, together with the rules for using cookies on our website.
Information clause – information for patients
- The controller of your personal data is NL Clinic Pierzyńscy Spółka Jawna.
- Personal data is processed for the purpose of providing a medical service on the basis of Art. 6(1)(c) and Art. 9(2)(h) of the General Data Protection Regulation.
- The recipients of the data will be persons authorised by the controller, processors with whom we have concluded appropriate data-entrustment agreements, and entities to which the data will be made available on the basis of the law.
- Personal data will be stored for the period required by law.
- You have the right to request access to your data and the right to rectify, erase or restrict its processing, the right to data portability, the right to object, and the right to withdraw consent at any time without affecting the lawfulness of processing carried out on the basis of consent before its withdrawal.
- You have the right to lodge a complaint with the President of the Personal Data Protection Office if you consider that the processing of your personal data infringes the General Data Protection Regulation of 27 April 2016.
- Providing your personal data is a requirement of the Act of 15 April 2011 on medical activity, the Act of 6 November 2008 on patients' rights and the Patient Ombudsman, and the Regulation of the Minister of Health of 9 November 2015 on the types, scope and templates of medical records and the way they are processed. You are obliged to provide it, and the consequence of not providing personal data will be the failure to provide the medical service.
Information for job candidates
In connection with Art. 13 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (GDPR), we inform you that the following rules apply to the processing of personal data.
The controller of your personal data is NL Clinic Pierzyńscy Spółka Jawna, ul. Obroki 68, 40-833 Katowice.
Purpose and legal basis of processing. Your personal data, to the extent specified in labour law, will be processed for the purpose of carrying out the current recruitment procedure (Art. 22¹ of the Labour Code; Art. 6(1)(b) GDPR), while other data on the basis of consent (Art. 6(1)(a) GDPR; Art. 9(2)(a) GDPR), which may be withdrawn at any time.
Data retention period. Your data collected in the current recruitment process will be stored until the end of the recruitment process and for 6 months after its completion, after which it will be destroyed.
Rights of data subjects. You have the right to:
- access your data and receive a copy of it;
- rectify (correct) your personal data;
- restrict the processing of personal data;
- erase personal data;
- lodge a complaint with the President of the Personal Data Protection Office (ul. Stawki 2, 00-193 Warsaw).
Information on the requirement to provide data. Providing personal data to the extent arising from Art. 22¹ of the Labour Code is necessary in order to take part in the recruitment procedure. Providing other data is voluntary. Your personal data will not be processed in an automated manner or profiled.
Information for employees

- Your personal data will be processed for the purpose of employment on the basis of fulfilling a legal obligation (Art. 6(1)(b) and (c) of the General Data Protection Regulation).
- The recipients of the data will be persons authorised by the controller and entities to which the data will be made available on the basis of the law.
- Personal data will be stored for the period required by law.
- You have the right to request access to your data and the right to rectify, erase or restrict its processing, the right to data portability, the right to object, and to withdraw consent at any time.
- You have the right to lodge a complaint with the President of the Personal Data Protection Office.
- Providing personal data is a statutory requirement of the Labour Code. You are obliged to provide it, and the consequence of not doing so will be the lack of employment.
Mandate contracts, contracts for specific work and contracts with self-employed persons
- The controller of your personal data is NL Clinic Pierzyńscy Spółka Jawna, ul. Obroki 68, 40-833 Katowice.
- Personal data is processed for the purpose of performing a mandate contract, a contract for specific work and/or a contract with self-employed natural persons on the basis of Art. 6(1)(b) of the General Data Protection Regulation.
- The recipients of the data will be persons authorised by the controller and processors with whom we have concluded appropriate data-entrustment agreements, and entities to which the data will be made available on the basis of the law.
- Personal data will be stored for the period required by law.
- You have the right to request access to your data and the right to rectify, erase or restrict its processing, the right to data portability, the right to object, and to withdraw consent at any time.
- You have the right to lodge a complaint with the supervisory authority.
- Providing personal data is a condition for concluding the contract. You are obliged to provide it, and the consequence of not doing so will be the failure to conclude the contract.
Cookies and external services
This website does not use its own cookies and does not automatically collect data about visitors for analytics or marketing. We do not use Google Analytics, advertising tools or tracking pixels.
To remember your decision about the external services described below, we store a single item in your browser's local storage (localStorage). It is not used for tracking and does not identify you as a person.
External services that require consent
On selected subpages we embed external services that may set their own cookies on your device. They load only after you give consent; until then we show a "Load" button in their place. These are:
- route map (Google Maps, provider: Google Ireland Limited);
- online appointment booking (ZnanyLekarz, provider: DocPlanner sp. z o.o.).
You can change or withdraw your consent at any time by clearing the site data in your browser or via the consent settings. Consent is not a condition for accessing the website's content.
Google Search Console
We use Google Search Console to monitor the website's presence in search results. It is a tool for the site administrator. It does not place cookies in visitors' browsers and does not track individual users.
